This form is provided for your convenience when planning a wedding reception with the Double Exposure Band.  Normally it's sent several weeks prior to the actual event.  The information you're providing will help your big day to go as smoothly as possible.  You might also want to print this form out on your own local printer after filling it out but before submitting it as a personal record.
